A form is a conversation with someone who would rather be doing something else. Every extra question, unclear choice or confusing layout makes people stop halfway or answer carelessly. These rules keep forms short and clear, and make the answers easier to use afterwards.

Before writing a question, decide what you will do with the answer. If you cannot name a use, leave it out. Short forms get more answers and more careful ones.
Mark a question as required only when the form cannot work without it. Too many required fields make people give up or type anything just to get through.
Use single choice when only one answer can be true, tick boxes when several can, and a rating scale for opinions or levels such as pain or satisfaction. Fixed choices are faster to answer and much easier to count than written answers.
Use a written answer only for things you cannot list in advance, such as a name or a comment. For ID numbers, codes and phone numbers, boxes with one character each reduce mistakes on paper.
Ask one thing per question and avoid words that push people towards an answer. Ask how satisfied they were, not how much they enjoyed their great visit.
Make choices cover every case and not overlap: add an Other or Not sure option when people may not fit your list.
Skip logic shows a question only after a certain answer, so people never see questions that do not concern them. A clinic form can ask about medicines only when someone reports a fever; an event form can ask for a talk title only from people who want to speak.
On paper, mark these questions clearly, for example: complete only if question 6 is Yes.
Many forms are still filled in on clipboards at a front desk or in the field. A printable version with bubbles to shade, boxes for numbers and a signature area makes paper answers easy to read and type in later.
Keep the digital and paper versions identical so both sets of answers end up in the same columns.
Fill in the form once as a respondent would, on a phone. You will find unclear wording, missing options and follow-ups that never appear. Then look at your test answers in a spreadsheet to check they are easy to use.
As short as it can be while still answering your question. Most everyday forms work best with 5 to 15 questions; longer ones should be split into short sections.
A 1 to 5 scale is quicker and enough for satisfaction or agreement. Use 1 to 10 when finer levels matter, such as a pain score.
Yes. FormForge builds a web form and a printable A4 sheet from one sentence or from your old form, with skip logic and an optional score or total. Building costs 1 credit and guests get 2 free credits every day.
FormForge's offline HTML form saves each answer on the phone or tablet it runs on and downloads them as a CSV file, so no sign-up is needed for the people filling it in.
