AI form builder

How it works

Describe the form, for example who fills it in and what you need to know, or add your old form as a PDF or Word file.
In Adjust, choose how it will be filled in, the look, and whether you want skip logic and a calculator.
Press Build form (1 credit). The digital form appears, and the Paper A4 view shows the printable version.
Edit questions for free, test it by filling it in, then download the offline form, the PDF sheet, CSV or JSON.

Tips for a better result

Say who fills in the form and what you will do with the answers.
If you have an old form, add it: every question is kept word for word.
Ask for a score or total only when you need one, such as a fee or a triage level.
Fill the form yourself once in the Digital view before sharing it.

Is it free?

Building a form costs 1 credit, and guests get 2 free credits every day (5 with a free account). Each AI change from the command bar costs 1 credit. Editing questions yourself, testing the form and downloading are free.

How do I collect answers?

Download the offline HTML form and open it on a phone, tablet or laptop. Each submitted answer is saved on that device, and the Download answers button gives you a CSV for Excel or Google Sheets. You can also print the A4 sheet.

Can it keep my existing paper form?

Yes. Add the old form as a PDF, Word or text file and every question and option is kept word for word, in the same order. FormForge picks the answer type for each and adds nothing unless you ask.

What is skip logic?

A follow-up question that appears only after a certain answer, for example asking which medicines someone takes only when they say they have a fever. It works in the web form and in the downloaded offline form.
